Terms of Service

HopeBridge Health Services LLC, doing business as Texas Care Baytown

Effective Date: January 1, 2026

About these terms

HopeBridge Health Services LLC is a Texas limited liability company that operates under the assumed name (DBA) Texas Care Baytown. In these Terms of Service, "Texas Care Baytown," "COMPANY," "we," and "us" all refer to HopeBridge Health Services LLC.

This website, home.texascare.info (the "Site"), is owned and operated by HopeBridge Health Services LLC. These terms apply to that site and to any other domain on which we host it. Our services are marketed and delivered under the Texas Care Baytown name, while contracts, billing records, and legal notices may appear under the legal entity name. Both names refer to the same business, and these terms apply to both.

By using the Site, you agree to be bound by these Terms of Service and to use the Site in accordance with these Terms of Service, our Privacy Policy, and any additional terms that apply to specific sections of the Site or to services available through it.

Accessing the Site in any manner, whether automated or otherwise, constitutes use of the Site and your agreement to be bound by these Terms of Service. We may change these Terms of Service or impose new conditions from time to time, in which case we will post the revised version on this website. By continuing to use the Site after we post changes, you accept the modified Terms of Service.

1. No medical advice, no provider relationship

The content on this Site is general information about our services. It is not medical advice, a diagnosis, or a treatment recommendation, and it is not a substitute for care from a licensed clinician.

Using this Site, or sending us a message through chat, email, or text, does not create a provider and patient relationship. That relationship begins only after intake is complete and services have been formally accepted. Nothing on this Site is a guarantee that you will qualify for services or for coverage.

Do not send detailed medical or clinical information through website chat, email, or text message. Those channels are convenient but are not secure. Use them to reach us, and we will move to a secure channel before discussing clinical details.

2. SMS messaging terms and compliance

Program description

This messaging program sends appointment confirmation and reminder messages to people who have contacted Texas Care Baytown through the chat on our website at home.texascare.info, or through our scheduling process, and have explicitly opted in to receive SMS notifications. Opt-in is collected through a clear, separate consent request at the point where you give us your mobile number. Messages include scheduling confirmations, appointment reminders, rescheduling updates, and customer support communications.

Cancellation instructions

You can cancel the SMS service at any time. Text STOP to the same number that sent you messages. We will confirm your unsubscribe by SMS, and after that confirmation you will no longer receive SMS messages from us. To rejoin, sign up as you did initially and we will resume sending messages.

Support information

If you have trouble with the messaging program, reply HELP for assistance, or contact us directly at [email protected] or +1 480-599-5781 during business hours.

Carrier liability

Carriers are not liable for delayed or undelivered messages.

Message and data rates

Message and data rates may apply to messages we send you and messages you send us. Message frequency varies based on your service usage and appointment schedule. For questions about your text or data plan, contact your wireless provider.

Supported carriers

Our SMS program works with all major U.S. wireless carriers, including AT&T, T-Mobile, Verizon, and most regional carriers.

Age restriction

You must be 18 years or older to participate in our SMS program. A parent or legal guardian may enroll and receive messages about services for a child in their care.

Privacy

For privacy-related inquiries, see our Privacy Policy.

We comply with all applicable laws and regulations, including the Telephone Consumer Protection Act (TCPA) and CTIA guidelines, regarding the use of SMS communications.

3. Intellectual property rights

Our limited license to you

This Site and all materials available on it are the property of HopeBridge Health Services LLC, doing business as Texas Care Baytown, or our affiliates or licensors, and are protected by copyright, trademark, and other intellectual property laws. The Texas Care Baytown name and logo are our marks. The Site is provided solely for your personal, non-commercial use.

You may not use the Site or its materials in a manner that infringes our rights or that we have not authorized. Unless we explicitly authorize it, you may not modify, copy, reproduce, republish, upload, post, transmit, translate, sell, create derivative works from, exploit, or distribute any material from the Site in any manner or medium. You may download or print one copy of individual pages for your personal, non-commercial use, provided you keep all copyright and proprietary notices intact.

Your license to us

By posting or submitting any material to us through the Site, internet groups, or other digital venues, including comments, photos, and videos, you represent that you own the material or have obtained the necessary permissions. You grant us a royalty-free, perpetual, irrevocable, non-exclusive, worldwide license to use, modify, transmit, sell, exploit, create derivative works from, distribute, and publicly perform or display that material.

This does not apply to protected health information or to anything you share with us in the course of receiving clinical services. That information is governed by our Notice of Privacy Practices and applicable law, not by this section.

4. Disclaimers

We may provide links and pointers to internet sites maintained by third parties. Linking to a third-party site does not imply endorsement or sponsorship of that site or of the information, products, or services offered on it.

The information and services offered on or through the Site are provided as is and without warranties of any kind, either express or implied. To the fullest extent permitted by applicable law, we disclaim all warranties, including implied warranties of merchantability and fitness for a particular purpose.

You agree to indemnify and hold harmless HopeBridge Health Services LLC, doing business as Texas Care Baytown, its affiliates, and their respective officers, directors, agents, and employees from any claims, causes of action, damages, liabilities, costs, and expenses arising out of or related to your breach of any obligation, warranty, or representation under these Terms of Service.

5. Third-party transactions

Certain sections of the Site may allow you to interact with or purchase from third-party vendors. We are not responsible for the quality, accuracy, timeliness, reliability, or any other aspect of those products and services. If you transact with a third party linked through the Site, information collected during that visit, including payment information, may be collected by both that merchant and us.

Your dealings with third-party vendors are solely between you and that third party. We are not responsible for any loss or damage incurred as a result of those dealings.

6. Registration and passwords

To access certain features of the Site, you may be required to register and create an account. You agree to provide accurate, current, and complete information during registration. You are responsible for keeping your login credentials confidential and for all activity conducted under your account.

If you suspect unauthorized use of your account, notify us immediately at [email protected]. We are not liable for loss or damage arising from your failure to meet this obligation.

7. Termination

We may terminate or suspend your access to the Site, without notice, if we determine that you have violated these Terms of Service or engaged in conduct we consider inappropriate or unlawful. On termination, you must stop all use of the Site and any content obtained from it. Termination of Site access does not by itself end any clinical services you are receiving, which are governed by your service agreement with us.

8. Governing law and dispute resolution

These Terms of Service are governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of law provisions. Any dispute arising under these Terms shall be resolved exclusively through binding arbitration seated in Harris County, Texas.

Nothing in this section limits any right you have to file a complaint with a state licensing board, a regulatory agency, or the Texas Health and Human Services Commission.

9. Changes to these Terms of Service

We may update these Terms of Service from time to time. The latest version will always be available at home.texascare.info/terms-and-conditions with the effective date shown at the top of this page.
