How it works
Getting Started is Easy
Step 1
Submit Your Information
Share your name, contact information, ZIP code, and who needs services. A Texas Care Baytown team member will reach out to guide you through the next steps.
Step 2
We Contact You
Our Intake Department will call to confirm Medicaid coverage and gather any details needed to begin the process for children, adolescents, or adults.
Step 3
Complete the Intake Forms
We will send you the required digital intake packet. You can complete everything online at your convenience.
Step 4
Clinical Assessment
A licensed clinician meets with you (or your child) to complete a mental health assessment. This helps determine the right level of care and which services are appropriate.
Step 5
Begin Services
Based on the assessment, clients may receive:
- Mental health rehabilitative services
- Skills training
- Community-based support
- Help with community support and resources
- Additional services based on individual need
Step 6
Ongoing Support
Your clinician and care team regularly monitor progress, update your plan as needed, and support you throughout your care journey.
